Chelsfield Surgery is a long established NHS GP practice serving the local community in Orpington and the surrounding areas. We aim to create a welcoming and supportive environment for people living with dementia by promoting continuity of care, clear communication and reasonable adjustments to meet individual needs.
As a GP practice, we play a key role in early identification, ongoing management and coordination of care for people living with dementia. We actively work with community services, social care and voluntary organisations to ensure patients receive holistic support.
